Question 1.
What do my spouse and/or dependents have to file with the Fund office after my death to be eligible to receive Survivors? Benefits?
All that is needed is a Death Certificate. Also, they must have been receiving benefits at the time of your Death.

Question 2.
How much does it cost for my family to receive this benefit?
Nothing. Your Family will be covered for benefits for (1) one year from the date of your Death.

Question 3.
After the one (1) year expires, will my family lose its cover-age?
It depends on whether your family elects COBRA Continuation Coverage, in which event, your family will have to pay a monthly premium to continue to be covered for benefits.
