1095 B Tax Documents: A copy of your 1095 B tax document is available upon request. If you would like
a copy, please contact the Fund office at (585) 424-3510 or send a written request to 3445 Winton Place,
Suite 238, Rochester, NY 14623. A copy of your 1095 B will be mailed to you within thirty days of your
request.
Attention all Local 9, 33, and 440 members.
- To access telemedicine, the members can call 1-866-692-5045 or visit https://www.excellusbcbs.com/find-a-doctor/telemedicine.
Telemedicine is for members who may not be able to see (urgent visit) or don’t have a current medical
provider. There will be no co-insurance for telemedicine visits.
- Example: Member has an earache, can call, and possibly obtain an Rx for antibiotics over the
phone.
- Members who have established doctors or mental health providers can have telephonic appointments without
any
co-insurance.
- EAP- Employee Assistance Program- 1-800-252-4555
- Counseling benefits;
- Pet Care resources; and
- Financial coaching.

Hello,
I hope everyone is doing well. I want to inform all of you that I will be retiring October 1. Laurie Good,
our Fund Accountant, will be stepping in as the new Fund Administrator. Laurie has been with our office
since 2007. She has plenty of experience in the various areas of our office and will do a great job as the
new Administrator. Please don’t hesitate to contact her if you have any questions or concerns.
I have enjoyed working with all of you over the years. It’s hard to believe that I have been with the Fund
Office for 39 years. It has been a pleasure getting to know you and being able to assist you with issues
that have come up. I hope I was able to make life a little easier for you.
I will miss the interaction with all of you and the Office Staff, but also looking forward to this new phase
of my life. My husband, Michael retired recently. We love the outdoors and the many things that it offers
and plan on traveling to different parts of the country. I enjoy photography so I can’t wait to get out
there and start taking pictures.
I wish all of you the best in the coming years. It has truly been a pleasure working with you.
