I hope all of you are enjoying your summer. It is going by too quickly!
The past couple of months have been busy at the Fund Office. There have been quite a few changes made to our health insurance plan due to the health care reform law. Changes have been made to the benefit dollar limit maximums and also to dependent coverage (up to age 26). For a complete listing of changes, please go to the Newsletters section of our website, click on the Welfare Fund Tab, then click on the Summary of Material Modifications (June 2011).
If you have a dependent between the ages of 19 to 26 and he/she is a full-time student, you will no longer have to provide a student certification form. To enroll your dependents (both students and non-students), you will need to complete the Special Enrollment Form for Eligible Adult Children Under Age 26. This form is located under the Forms section of our website (Welfare Tab). Please note: If you would like to have your dependent covered under the Local 33 & 440 Supplemental Fund, you will need to complete a separate enrollment form for this Fund. These forms are also available on our website.
Welfare Fund Participants: To reduce your out-of-pocket medical expenses, use participating healthcare providers in the MultiPlan/PHCS Network. For a listing of participating providers, go to www.multiplan.com or call (888) 987-7427.
Pension Fund Participants: When thinking of retirement, please contact Sue Garrad to obtain an estimate of your benefits. You should contact Sue two (2) months before the month you intend to retire.
