It is now Open Enrollment for the Iron Workers District Council Welfare Plan. During this time, you have the opportunity to make changes to your existing health insurance coverage.
From now through December 31, 2014 you have the ability to:
- Add/Remove Dependents;
- Change Your Level of Coverage; or
- Opt Out of the Plan.
You are REQUIRED to complete the COB/Enrollment form (yellow form) even if you're not making any changes. Due to the Affordable Care Act, we are required to have social security numbers for all dependents on your plan. This includes your minor dependents.
If you don't complete and return the COB/Enrollment form by December 31, 2014, claim payment(s) will be delayed until the Fund Office receives a completed form.
**If you currently have Basic Coverage and do not return a completed COB/Enrollment form, your level of coverage will be changed from Basic to Single with $0.50 being rolled into the Annuity Fund effective January 1, 2015. You will remain at the Single level of coverage until 2016.
|Below is a description of each option available to you:
Select this option if you are enrolled in other group health coverage, which meets the minimum value requirements of the Affordable Care Act, and do not want health and dental benefits under the Plan. You will have:
- $5.90/hr. contributed to the Welfare Plan;
- $1.50/hr. will be contributed to your Supplemental or Annuity Plan.
Select this option if you want health and dental coverage and do not have any eligible dependents. You will have:
- $6.90/hr. contributed to your Welfare Plan;
- $0.50/hr. will be contributed to your Supplemental or Annuity Plan.
Select this option if you want coverage for you and your eligible dependent(s). You will have:
- $7.40/hr. contributed to your Welfare Plan;
- $0.00/hr. will be contributed to your Supplemental or Annuity Plan.
For Local 9 & 33 Members:
WRA Benefit Changes (Members Over the Age of 50)
Listed below are the recent changes to the Wage Replacement Account (WRA) Benefits. These changes are effective for claims submitted after November 1, 2014. Please note that claims can be submitted for time periods prior to November 1st. However, the claim period must fall within the one (1) year filing time limit. All benefits are now the same for those over the age of 50 as they are for those under the age of 50. The only exception is the vacation benefit, which remains at the higher benefit level.
Listed below are the gross benefit amounts. The net benefit amounts can vary from person to person depending on an individual’s exemptions.
WRA Benefit Changes
Under age 50: $300 per day, $1,500 weekly gross benefit (2 weeks (10 days) max per yr)
Over age 50: $900 per day, $4,500 weekly gross benefit (6 weeks (30 days) max per yr)
$800 weekly gross benefit - Waiting week
$400 weekly gross benefit - Subsequent weeks
$500 weekly gross benefit
Disability (Workers’ Compensation):
$400 weekly gross benefit
