Create a member account at eyemed.com. Everything is right there in one spot. Check claims and benefits see special offers and find an eye doctor — search for one with the hours, location and brands you want. For maximum mobility, try the EyeMed Members App (Google Play or App Store).

You can also print an ID card and download an Explanation of Benefits. It’s also available on the EyeMed Members App through the App Store or Google Play.

Within the provider network

Your annual eye exam, single vision, bifocal and trifocal lenses are fully covered with $0 copay. Other services have a copy or are covered 100% up to your allowed maximum. In-network providers submit their claims directly to EyeMed, making it easy for members. Visiting in-network providers will ensure with the lowest out-of-pocket costs.

Outside the provider network

When you visit an out-of-network provider, you will need to refer to your benefit summary for the allowable amounts. All out-of-network providers will require you to pay the full amount at the time you receive care. You then submit an out-of-network claim form with a picture of your itemized bill to EyeMed for your reimbursement.
