The purpose of the Retiree Plan is to:

1. Subsidize eligible pre-65 retirees for the cost of health insurance premiums up to $750 per month
2. Offer medical, dental, vision and prescription benefits to both eligible pre-65 and post-65 retirees

Eligibility for individual participants shall be determined according to the provisions of the MOU between the various Unions and Employers participating in the Trust, along the rules described below. Coverage will become effective on the first day after active coverage ends due to retirement.

Eligible Retirees must complete a Retiree Plan Enrollment Form to enroll themselves and/or their eligible Surviving Spouse. Neither you nor your Surviving Spouse will be eligible for benefits until you have submitted a completed Enrollment Form to the Trust Administration Office and have been notified that your enrollment is complete, and your participation has been approved.

Note: Dependents will not be eligible for coverage.

Medical Expense Reimbursement Plan

The Retiree Plan will provide a source of funds to reimburse you for premiums which are not otherwise reimbursed. These expenses must be incurred while the Retiree Plan remains in effect, you are eligible, qualified, and participate in the Retiree Plan. The Medical Expense Reimbursement Plan is a recordkeeping reimbursement account the Trust will establish on your behalf. Each month, the Trust will credit up to $750 to your MERP to help cover the cost of eligible health care ex-penses for you.

The Trust has partnered with Via Benefits Insurance Services to help you make a funding decision and/or a healthcare election. You will have access to concierge service and begin to shop for and enroll in a new medical plan. Via Benefits is an unbiased resource that will help you navigate your health care choices and provide guidance as you choose coverage. Services include:

• Medical Expense Reimbursement Plan (MERP)
• Helping you find and enroll in health insurance coverage(s)
• Offering support and guidance throughout the year
